
External Laboratory Analysis Request Form
External Customers
External customers issues test requests by submitting SDM-H-00-LAB-PRO-0002-FRM-003” External analysis request form” through e-mail. All communication and evaluation of the requests are executed under this particular form.
Below areas are critical to indicate in the form.
- Clear description of the analysis sample;
- Test methods requested by the customer;
- Number of samples;
- Sample details (sample amount, sample container feature, etc.)
- Possible risks associated with the sample.
- Request for conformity assessment (pass/fail, within/out of tolerance, etc.) according to a standard or specification
- Request for Laboratory decision rule application
- Requesting for measurement uncertainty
- Language of the Test Report
Note on Decision Rule
If there is a defined decision rule mandated by the regulation, Laboratory shall apply on this.
Otherwise, if the decision rule is not defined or determined in any specific regulatory document, Laboratory applies the simple acceptance decision rule agreeing with the customer.
The measurement uncertainty value is evaluated according to the simple acceptance decision rule and the results are reported without adding or subtracting the measurement uncertainty value.
When another condition is requested by the customer, Laboratory agrees with customer and applies the determined decision rule.
The Laboratory evaluates the requests according to the:
- Conditions of the laboratory;
- Suitability of the equipment;
- Stock status of reference/consumables and other necessary materials;
- Availability of manpower
